Signal conditioning includes:
Signal isolation:
Protects sensitive equipment from potential hazards that may come through the signal path from the sensor.
Preprocessing: Includes signal preprocessing and removal of undesirable signals.
Linearization: Converts a non-linear input signal to a linear output signal. This is necessary when sensors produce signals that are not linearly related to the physical measurement.
Amplification: Increases the input signal strength or amplitude. This can increase the resolution of the measurement.
Analog-to-digital conversion (ADC): Changes a continuously variable, or analog, signal into a multilevel digital signal without altering its essential content. 

Signal conditioners accept inputs such as:

AC voltage and current
Frequency
Electric charge
