Common traps: using a modifier to “force payment” without documentation, confusing -25 vs -59, and mixing -TC/-26 incorrectly.

Worked example (mini):
Same day: patient has an office visit for new symptoms + a minor procedure.
 If E/M is significant and separately identifiable beyond the procedure’s pre/post work → E/M + modifier -25.
