A 37 year old man sees GP due to progressive SOB and vague right upper quadrant pain. He is unable to walk up two flights of stairs without losing his breath. He has smoked a half pack of cigarettes per day for 15 ears. Temp is 99.7, HR is 72, resp rate is 22. Physical exam shows clubbing of fingers, scleral icterus, generalized jaundice, hepatomegaly. Lung auscultation reveals diminshed breath, voice and heart sounds. Lab studies show an ALP of 85, ALT 225, AST 200. Total bilirubin of 3.3 direct bilirubin of 3.0. He undergoes a liver biopsy. Histology shows presence of hepatocytes of large numbers of eosinophilic globular cytoplasmic inclusions that stain with periodic acid schiff stain. There is also mild to moderate fibrosis of the portal triads. 1. What pathology do you suspect and why?
