Cause= developmental insufficiency of surfactant production and structural immaturity in the lungs, or any other trigger (eg pneumonia) for a systemic inflammatory cascade (irds the leading cause of death in premmies) the ground glass appearance is caused by increased vascularity, while air bronchograms are caused by air in the bronchi contrasting with fluid in the alveoli. The deterioration is likely to be caused by pulmonary interstitial emphysema and possibly even pneumothorax. Chronic lung disease develops as a sequence from ards with ventilator induced lung injury causing airway stretching, alveolar membrane distortion, and intercellular junction rupture in the more compliant alveoli. The resulting microscopic barotrauma and volutrauma allow leakage of proteinaceous edema into the alveoli causing the course reticular pattern often seen in cld.
