(decreased use with paracetamol and nsaids as baseline medications) opioids are used for moderate to severe pain. Where possible oral opioids should be used as kids will deny they have pain to avoid needles. Oxycodone has much better bioavailability so is preferred to morphine. Tramadol also better. Iv opioids and infusions (pca, nca and continuous) used for closer pain control if rapidly changing, but require closer monitoring. Pca requires patient age atleast 5-6. Nca is used when there is a large component of â€˜incident painâ€™ e.g. pain on movement or coughing. Side effects= respiratory depression: have naloxone available (initial dose 1-5 micrograms/kg). Vomiting: before using anti-emetic drugs always ensure that patients are adequately hydrated and analgesed and that hypoglycaemia and hypotension are not causative factors. Itch: before using anti-pruritic drug ensure general skin care is optimised i.e. change old dressings, adequate skin washing, use skin moisturisers. Ensure that pruritus is not the result of uraemia or hyperbilirubinaemia. Constipation: patients receiving long-term opioids do not become tolerant to the constipating effects of opioids. Ensure that patients are well hydrated at all times. Prescribe a laxative if needed.
