Diagnosis more difficult in the young due to non-specific presentations. There may be drowsiness/irritability/high pitch cry (altered loc), vomiting, fever/hypothermia, apnoea, seizures. In any sick infant in this age group a high index of suspicion must exist in order to allow earliest possible diagnosis and treatment. In contrast, symptoms become more cns specific in older children to accompany the sick appearance (and possible fever). Neck stiffness is present in 70%, seizures in 30%. Kernigs sign and brudzinskis sign are signs of neural irritation on stretch (slr and neck fn respectively). Photophobia suggests complications like venous sinus thrombosis. Do not discount non-specific signs of changed loc/irritability and anv (anorexia/nausea/vomiting) prior antibiotics may lead to delayed diagnosis, because there is often less fever/changed loc, more vomiting and the rate of positive cultures/g stains is reduced (altho other areas of csf not affected) risk factors= age less than 5, no vaccination to pneumococcus or meningococcus b, recent infection, contact with someone with meningitis, recent travel, drug allergies, recent antibiotics. If younger than 3 months maternal obstetric history is imporant to check group b strep colonisation. Complications= septic shock, dic, cerebral herniation
