Respiratory- lungs, ear, nose and throat for pneumonia, viral, otitis media. Heart- check for heart murmurs infective endocarditis. Cns- meningitis urinary- uti a full infection screen is aka full septic workup (fsu)= blood c&s;, cbc and differential, urine r&m;, c&s;, lumbar puncture, cxr if respiratory symptoms, stool c&s; if gi sxs. The threshold for intervention and fsu is derived from the clinical assessment of the child as young (less than 3 months) or 'toxic' if over 3 months (sick and poorly perfused). If the child is non-toxic with a high-fever (over 39.5) urinalysis and fbc are done. If no high fever (less than 39.5) then urinalysis done with observation and follow up.
