- a child less than 3 months of age is more likely to have a structural airway problem. A child between 1 and 3 years with acute onset respratory difficulty without fever may have an inhaled foreign body. The combo of expiratoy and inspiratory stridor increases the likely hood of underlying fixed tracheal obstruction. - assessment of severity- classed as mild. moderate or severe to guide treatment using algorythm. Mild= mild chest wall retractions and mild tachycardia, but stridor at rest will not be present, and generally appears happy and interested in surroundings. *no pharm treatment necessary. Moderate= persisting stridor at rest, chest wall retractions, use of the accessory muscles of respiration and increasing heart rate. *the child will need systemic corticosteroids and observation for a minimum of four hours. If the child continues to have stridor at rest, then further treatment will be considered. Severe= as above plus child change in behaviour eg) restless, agitation, looks ill/cyanosed. *immediate application of mask oxygen with further nebulised adrenaline as preparations are made to intubate the child by someone skilled in paediatric intubation, systemic steroids once airway secured. Not supported by literature= oximetry is not well correlated with severity of croup, lateral airways xray/ chest xray unncessary to confirm diagnosis croup is a clinical diagnosis (in the absence of additional findings eg) crackles on auscultation), no support for mist therapy, and clinical scoring systems are of limited value
