History- mechanism inconsistent/late presentation, home/living situation unstable. Physical- general appearance overly affectionate/fearful, bruises not on legs (eg abdomen) and a pattern matching abuse (eg shape of fist), burns eg) cigarette burns, neuro exam retinal haemorrhage suggests shaken baby, sexual abuse if suspected may have overt or very subtle signs on inspection of anus/genitals. Investigations- aptt/pt/fbc to rule out heamatological causes of excessive bruising, xrays- ribs, spiral (twisting) and metaphyseal chips (type of salter-harris fracture that suggests traction/twisting force), pregnancy testing and sti cultures for sexual abuse. There is a mandatory duty to report any current and reasonable concerns that the child is at risk of harm to docs. You must discuss this with colleagues and be able to articulate your reasons for your suspicion.
