Intermittent severe abdominal pain- screaming child for bouts of 2-3 mins bliary vomit red current jelly stools- due to blood it is telescoping of the bowel which most commonly occurs in the ileum to the transverse colon. Most common in 1st year of life. The cause of intussusception is thought to be from inflammation that causes enlargement of the payers patches in the bowel wall (lympatic tissue), which acts as a lead point and gets dragged into the lumen in a telescoping action with peristalsis. Ultrasound imaging is quite sensitive and specific for intussusception, whereby there is a 'target sign' representing bowel within bowel when viewed end on. Intussusception is treated successfully in 80-90% of cases using an air hose to perform an 'air enema' which pushes the bowel back (like blowing up a rubber glove finger). Done under ultrasonographic guidance as both an investigation and treatment. Only contraindication= peritonitis and septicaemia.
