Laryngotracheobronchitis= croup this is inflammation of the airways of the larynx, trachea, and bronchi that cause subglottic obstruction and therefore lead to cough and stridor. It is more severe if the stridor occurs at rest, or if the noise stops as the degree of obstruction worsens. The 2 types are thought to be distinct entities. Acute viral croup is precipitated by viral infection (influenza 1 and 3 and rsv), and usually spans the first 2-3 days of a 7-10 day infection. Recurrent croup (spasmodic croup) can occur independently of viral infections, but is associated with asthma and a family history of atopy- thought to be a manifestation of airways hyperresponsiveness. Peak incidence is 5 per 100 in the second year of life. Inhaled corticosteroids are useful to limit progression and hospitalisation, but nebulised adrenalin is faster acting and is as effective so is commonly used, especially in severe cases. If necessary intubation can be done and bronchoscopy is reserved for cases that are atypical in their course.
