1. Ecgs: What changes do you get with significant left ventricular hypertrophy?

Wiper inspection: varicose eczema lipodermatosclerosis (atrophy and loss of elasticity of the skin and subcutaenous tissues) skin pigmentation (brown due to haemosiderin deposition) ulceration (active or healed - latter causes white patches called atrophie blanche) scars stand patient up and varicosities along the long saphenous and short saphenous veins. Try to decide which veins are involved, long saphenous, short saphenous or calf perforating veins. Palpation: saphena varix at the saphenofemoral junction (find femoral artery between asis and pubic symphsis, vein is medial and saphenofemoral function is approx 2 fingers breadths below inguinal ligament) if present perform cough test. 'toniquet' test or trendelenberg's test. Idea is that you compress the superficial veins. Start high on thigh as possible, empty leg by getting patient to lie down and raise leg, apply tourniquet, stand patient up, if torniquet stops filling then vv were caused by saphenofermoral junction reflux, if does not then there is short saphenous or calf perforator incompetence. Move to just below knee and repeat. I would use duplex ultrasonography, where availible, to confirm the site of incompetence. To complete my examination i would also perform a tap test, auscultate the vein for bruits (indicating the presence of arteriovenous fistulae) and examine the abdomen for masses (including a dre) to ascertain whether the varicose veins are primary or secondary.
1.Decreased Arterial oxygen saturation e.g decreased atmospheric pressure, impaired pulmonary function, anatomic shunts 2.Haemoglobin Abnormalities e,g methemoglobinemia
Distress, anxiety, pallor, sweatiness. Increased or decreased pulse and bp. 4th heart sound. Signs of heart failure (raised jvp, 3rd heart sound and basal crepitations) or a pansystolic mumour.
Tall r wave greater than 25mm in v5 or v6, deep s wave in v1 or v2. R waves in lead v5 or v6 plus s waves in lead v1 or v2 greater than 35mm. T wave inversion in i, vl, v5 and v6, sometimes v4 and l axis deviation.
