A 59 year old man is cyanotic post operative after complete bilateral lung transplant 10 hours prior. Supplemental oxygen has been increased form 40 % to 100 % over last hour. Diffuse crackles are heard over both lungs. Trans thoracic echocardiogram shows normal left ventricular function. Chest x ray shows diffuse opacities throughout bot lungs. 1.
