  • Fix: Place-value chart; say & trade: “5 tens + 6 ones”; build with base-ten blocks.

  • Check: True/False → 7+3 = 5+5 ; 12 = 9+4.
  • Fix: Reframe “= as ‘same as’.” Use balance scale; write two ways to make 12.

[FractionsMis] (adds across / compares by denominator)

  • Check: 1/3 + 1/3 = ? (watch for 2/6) · Which is larger: 3/8 or 3/5?
  • Fix: Same-denominator/ same-numerator routines on number line; fraction strips; rename to common denom.

  • Check: Compute 3.4 + 0.56; 5.2 − 0.89.
  • Fix: Line up decimal points; annex zeros; estimate first (“about 3.9”).

  • Check: A rectangle 5 cm by 3 cm → perimeter? area? (look for cm² vs cm swap).
  • Fix: Write unit before solving (“perimeter: ___ cm”); unit ladder (cm ↔ m; cm² ↔ m²).

  • Check: Bar graph with scale of 2s—ask “How many more?” across unequal bars.
  • Fix: Pre-flight: Title • Axes • Units • Scale • Legend; have student restate the scale aloud.

  • Check: Two-step word problem; ask them to draw or bar-model before computing.

Fix: “Read → Draw → Equation → Answer w/ unit.” Require one sentence answer.
